Necessary Forms for Recharter

Annual Memorandum of Understanding- This form must be signed by the sponsoring organization head.

Youth Application- Each participating youth must be included on the youth roster.  It is ideal for youth to add their names to the roster at the Post Open House.  The application must then be signed by the Post Advisor.  (A parent/guardian signature is also required if the youth is under 18 years of age.)

Adult Application- Each participating adult must be included on the adult roster.  Each Post must have a minimum of four adult advisors, and background checks will be conducted on adult participants.

Youth Protection Training- The completion of the Youth Protection training with verification by a signed certificate is necessary for all adult participants.  The certificate will be held on file for two years, when the training must be completed again.

Background Check Waiver- Use this form if your participating organization is a government agency that requires background checks as a condition of employment.  This form certifies that a background check has been completed for the individual on the form.

*Other documents may be necessary for re-chartering a Post.  Your Exploring Executive will assist you with re-charter paperwork.

Exploring Adult Leader Guide- This guide outlines the roles and responsibilities of adult leaders.

Exploring Youth Leader Guide- The purpose of this guide is to help youth understand what it means to be a leader in an Exploring Post.  It answers questions about effective leadership, responsibility, and planning.

Open House Guide- Use this guide to help plan a successful Post Open House.  It includes the steps to organizing an Open House, as well as sample letters inviting students to attend.

Sample Agenda for a Post Meeting- Use this outline to organize meetings.

Adult Resource Survey- Use this form to discover the talents and resources of the adult Post leaders and sponsoring organization.

Youth Activity Interest Survey- Let youth fill out this form to help decide what type of programs they would like to see the Post conduct.

Program Match Sheet- Once youth have completed the Youth Interest Survey, use this sheet to organize the information and to help you better decide which activities to plan.

Local Outing Permit- For trips and camps under 500 miles

National Outing Permit- For trips over 500 miles and outside the US

Money Earning Application- Fill out this form prior to a Post’s money-earning event

Permission Slip- Parents and/or guardians can sign this form to approve youth outings.
